Please acknowledge your understanding and willingness to abide by the Code of Conduct by reading and then signing this form.

1.      I will encourage good sportsmanship by demonstrating positive support for all players, coaches and officials at every game, practice or other youth sports events.

2.      I will place the emotional, physical and moral well being of my child and all of his/her teammates ahead of any personal desire to win.

3.      I will insist that my child play in a safe and healthy environment.

4.      I will provide support for all coaches and officials working with my child so as to maintain a positive enjoyable experience for all.

5.      I will demand a drug, alcohol and tobacco-free sports environment for my child and agree to assist by refraining from their use at all youth sports events.

6.      I will remember that the game is for the players and not for the adults.

7.      I will do my very best to make youth sports events fun for my child.

8.      I will ask my child to treat other players, coaches, fans and officials with respect regardless of race, sex, creed or ability.

9.      I will promise to help my child enjoy the youth sports experience within my personal constraints by being a respectful fan, by providing transportation or whatever I am capable of doing.

10. I will support the team by having my child on time to practices and games.

11. I will NOT engage or support my child in any unsportsmanlike behavior, such as disrespecting the coaching staff, using obscene language and profanities, encouraging fights, abruptly withdrawing from tournaments and the like.

12. I will support the decisions of the coaching staff on matters of selection of team players, staff members and other related administrative decisions for the team.

13. I will support all financial requirements needed by my child to participate in all related activities of the team.

I have read and fully understand all components of this Code of Conduct and agree to abide by all the provisions set forth above.  I understand that failure to comply with this Code of Conduct may prevent my child from participating in any JSAG Basketball activities and removal of my child from the team. I understand that I may be asked to leave the organization if my or my child’s behavior is having a negative impact on the players or other fans.
